Auf der Vorderseite sind alle persönliche Daten von Franz Waldhauser vermerkt, die Tabelle war wie bei jedem Formular bereits vorgedruckt. Auf der Rückseite sind verschiedenen handschriftliche Vermerke sowie ein Stempel in rot, Druckbuchstaben in blau und Signaturen.
